- Starting at $90/month for individuals | $200/month for families (up to 4 members)
- Individual membership includes up to 10 healthcare appointments per year and offers a reduced rate for any additional appointments beyond the 10.
- Family membership includes 20 visits per year, which can be used by any family member. Additional appointments beyond the 20 are available at a reduced rate. Sports physical examinations for children are free and do not count towards the 20 visits.
- Concierge Care: The ultimate in personalized healthcare and wellness.
- Individual Membership: Starting at $300/month

Understanding Your Visit: Costs and Coverage
At Cossmedic Wellness and Aesthetics, it's important to be aware of the typical costs you may incur during a visit with your Primary Care Provider and the services not available at our clinic.
Self-Pay Clinic We operate as a self-pay clinic. This means we do not participate in any health insurance plans, HMO panels, or third-party payors. We do not submit bills or seek reimbursement from third-party payors for services provided.
Direct Primary Care (DPC) Program Please note that our DPC program is not an insurance policy. It provides access to routine primary care appointments with our providers, both in person and through telehealth. Enrollment does not include prescription medications, diagnostic costs (i.e., imaging and labs), hospitalization, or emergency room visit coverage. We recommend maintaining an insurance policy if such coverage is required.
Payment Options You can use your Health Savings Account (HSA) or Flexible Spending Account (FSA) to pay for your care. Please consult your employer or health insurance representative for details. We accept cash and most major credit cards for a small fee.
