Department of Transportation physicals examinations is required to maintain a valid commercial driver’s license (CDL). The DOT physical consists of completing a medical history form, which is reviewed by your provider, and a physical examination. If your employer requires specific medical paperwork in addition to those of the DOT, please bring the form (s) to your appointment to be completed.
